I agree to disclose any known allergies or sensitivities related to my child prior to the party and will notify of any updates. While reasonable precautions are taken, I acknowledge that Pretty Pink Pamper Parties cannot guarantee a completely allergen free environment, and I agree not to hold the business liable for any allergic reactions or adverse effects. I agree to disclose of any skin conditions which might jebadize from taking part -like warts or verrucas that could pose a risk to others during treatment.

I understand that the event includes a variety of pampering activities such as: manicure nail polish, pedicure nail polish, foot bath, nail art stickers, cucumber eyes and mini hand and foot message.

The products used on the day:
Rimmel nail varnish 60 second - Nail varnish removal if needed - Cucumber on eyes - Sentive foot soak for feet - Hand and feet cream for massage.

By signing this form, I voluntarily assume all risks associated with my child’s participation and agree to release, waive, and hold harmless Pretty Pink Pamper Parties, from any and all liability for injury, loss, or damage, including personal or property loss, that may arise during or as a result of the event.
